  • This Evaluation is the most Comprehensive and includes: Cognitive (IQ) Testing, achievement testing, surveys assessing psychosocial functioning, and diagnostic clinical interviews. The fully psychoeducational evaluation is the best way to determine: a) Learning strengths and areas of challenge, b) whether you meet criteria for a psychological disorder, and c) how learning abilities and psychological symptoms may influence one another. Information collected during the evaluation, diagnostic determinations, and tailored recommendations are summarized in a report that is shared with caregivers.

  • This Assessment is purely an assessment of cognitive capabilities. Cognitive assessment broadly covers an individuals abilities in five areas: attention, memory, visuospatial, language, and reasoning. This assessment is useful for those who want a general understanding of their cognitive functioning and recommendations for either future testing and/or broad information about relative strengths and weaknesses. psychosoc

    • Learning and Educational Experiences Interview (with individual/child/parent/teachers etc.)

    • Assessment Report that Summarizes Testing Results and Provides Recommendations

  • This evaluation assesses cognitive ability (IQ), achievement level and ADHD-related difficulties Most caregivers/individuals seeking this evaluation have concerns about specific learning difficulties and/or possible ADHD. An educational evaluation is best suited for individuals who are not experiencing moderate to severe mood, anxiety or conduct problems (in this care, a full psychoeducational evaluation would be more appropriate).

    Typically, educational evaluations include:

    • Intake and Assessment Planning

    • Cognitive Testing

    • Achievement Testing

    • Learning and Educational Experiences Interview (with individual/child/parent/teachers etc.)

    • ADHD and Adaptive Functioning Surveys (as needed)

    • Functional Measure of Assessment

    • Assessment Report that Summarizes Testing Results and Provides Recommendations

    • Feedback Session

  • This Evaluation assesses mood, anxiety, behavioral, social, and personality difficulties. It also includes an assessment of factors that might be worsening existing psychological difficulties as well as recommendations for specific therapeutic interventions that would be most helpful. This evaluation is most helpful for those seeking out diagnostic clarity for psychological issues. This evaluation is best suited for those who do not have a history of learning/academic/attentional issues (for these issues, a full psychoeducational evaluation would be most appropriate).

    Psychological Assessments include:

    • Intake and Assessment Planning

    • Self-, Caregiver-, Teacher- report questionnaires

    • Psychological and personality Diagnostic Interviews

    • Psychological symptom and adaptive functioning surveys

    • Assessment report summarizing testing results and recommendations

    • Feedback sessions

  • Depending on the questions you have and the clinicians judgement, other assessments may be added on to give you a more complete view of what is going on. These services will be discussed with you should the clinician believe them to be clinically appropriate and will only be added on with your consent.

    The following are options for add on assessments:

    • Oral Language: An in-depth assessment of oral language abilities can be helpful if you or your child have specific difficulties with reading and/or writing.

    • Memory: A comprehensive assessment of memory can help if you or your child are struggling with specific memory issues (long-term, short-term, being forgetful, etc.)

  • We are able to provide the following assessments which are for particular services and vary in terms of session length and pricing. Please reach out to our team for more information about any of the following: Bariatric evaluations, emotional support animal letters, and any others not listed here.

Scheduling and Pricing

You will be assigned an intake with either Dr. Diaz herself or one of her colleagues. After the initial intake session, your clinician will share their plan for testing, which will include a breakdown of services to be completed and an estimated number of sessions. We will also provide you with an estimate of any out-of-pocket costs. See below for the breakdown of time and costs for our various types of evaluations.


Psychoeducational Assessment

A full evaluation often requires 5-7 sessions, some of which can be scheduled on the same day

Self-pay rate is estimated to be between $3,500-4,500



Educational Assessment

An educational assessment typically requires 3-4 sessions, some of which can be scheduled for the same day.

Self-pay rate is estimated to be between $2,000-3,000*

Psychological Assessment

An psychological assessment typically requires 3-4 sessions, some of which can be scheduled for the same day.

Self-pay rate is estimated to be around $2,000-3,000*





Cognitive Assessment

A cognitive assessment typically requires 2-3 sessions, some of which can be scheduled for the same day.

Add on Assessments

Add on assessments typically require 1-2 sessions, some of which can be scheduled for the same day.

Self-pay rate for Add ons are estimated to be around $650-1,000*


*Rates are based on the scope of service. As clinician learns more during the assessment process they may recommend additional testing or evaluation to further evaluate specific deficits or symptoms. These additional services and the additional costs of theses services will be discussed with you prior to the services being provided
